Insurance cards should be available at each visit. If you have not yet received your card, please contact your carrier and bring the following information:
- Group number
- ID/policy number
- Mailing address for claims
If you cannot provide the necessary information for filing your claims, you will be expected to pay for the charges at the time of service.
Co-pays are due at time of service.
Any non-covered charges are your responsibility. If you have questions regarding your policy benefits, contact your insurance provider prior to your child’s visit.
Balances are due upon receipt of the first statement. Payments may be made by cash, check or credit card. We accept Visa, MasterCard or Discover.
There will be a fee of $25 for all returned checks.
In cases of divorce and/or separation, the legal guardian and/or the person bringing the child in for services will be held responsible for paying any balance. Legal documentation must be provided stating that someone other than the legal guardian is financially responsible, including name and all billing information for that person.
